What this means for you: When you open an account with us, we will ask for your and/or your business's legal name, address, date of birth, tax identification number, and other identifying information that will assist us with identifying you and/or your business. We may also ask to see your photo identification (driver's license) or other identifying documents. For businesses, we may ask for copies of certified articles of incorporation, an unexpired government-issued business license, a partnership agreement, or other documents that indicate the existence and standing of the entity.
We may change APRs, fees, and other Account terms in the future based on your experience with U.S. Bank National Association and its affiliates as provided under the Cardmember Agreement and applicable law.

Fixed Payment Programs: From time to time we may offer to you the benefit of our Fixed Payment Programs, including ExtendPay® Plans and ExtendPay® Loans, which allow you to pay off balances in fixed monthly payments over time. See your Cardmember Agreement for more information.
Balance Transfer Information: Balance transfer transactions from other U.S. Bank National Association accounts are not permitted. You may cancel a balance transfer request within 10 days of account opening by calling 800-285-8585. After 10 days from account opening, requests to stop payment on Balance Transfer Check(s) issued by U.S. Bank shall not be honored unless the check(s) has been stolen, lost or destroyed.
Rewards are earned on eligible Net Purchases. Net Purchases are Purchases minus credits and returns. Not all transactions are eligible for rewards, such as Advances, Balance Transfers and Convenience Checks. Upon approval, please refer to the materials received with your new card and the Program Rules in the online Rewards Center by logging in to usbank.com for additional information. Account must be open and current (that is, no Minimum Payment is past due) on the closing date of the billing cycle in order to earn and redeem FlexPoints. Merchants identify their business by selecting a merchant category code established by Visa. A Purchase will not earn additional FlexPoints if the merchant's selected code is in a category that is not eligible for additional FlexPoints earning. U.S. Bank does not determine the category codes that merchants choose and reserves the right to determine which Purchases qualify for additional FlexPoints. We reserve the right to adjust the number of FlexPoints for Purchases or to stop issuing FlexPoints for Purchases on the Account, upon notice to you. U.S. Bank FlexPerks Gold American Express Cardmembers will earn one (1) Point for every $1 in eligible Net Purchases. In addition, you will earn additional Points for the following Net Purchases: You will earn four (4) additional FlexPoints for every $1 in eligible Net Purchases spent on prepaid car and hotel reservations in the online FlexPerks Rewards Center using the U.S. Bank FlexPerks Gold American Express Card. Please allow 1-2 statement billing cycles for bonus FlexPoints to appear on the credit card statement. Refer to the Rewards Center for full details. You will earn two (2) additional FlexPoints for every $1 in Net Purchases during each billing cycle for any merchant classified as a restaurant. Purchases made at a restaurant may not be recognized as a restaurant and will not qualify for additional Rewards.
