IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT

To help the United States fight the funding of terrorism and money laundering activities, U.S. law requires U.S. Bancorp, like other financial institutions, to obtain, verify, and record information that identifies each customer that opens an account.

What this means for you: When you open an account with us, we will ask for your and/or your business's legal name, address, date of birth, tax identification number, and other identifying information that will assist us with identifying you and/or your business. We may also ask to see your photo identification (driver's license) or other identifying documents. For businesses, we may ask for copies of certified articles of incorporation, an unexpired government-issued business license, a partnership agreement, or other documents that indicate the existence and standing of the entity.

We may change APRs, fees, and other Account terms in the future based on your experience with U.S. Bank National Association and its affiliates as provided under the Cardmember Agreement and applicable law.

When you make a payment, the amount up to your Minimum Payment is applied first to the monthly payment obligation for U.S. Bank ExtendPay® Plans and U.S. Bank ExtendPay® Loans if any, and then to non-Fixed Payment Program balances in the order of the lowest to highest APR. Any amount over your Minimum Payment is applied to balances in the order of highest to lowest APR.

How to Avoid Paying Interest on Purchases Your due date is 24 – 30 days after the close of each billing cycle. We will not charge you interest on new Purchases if you pay your entire balance, or Plan Adjusted Balance if you have any Fixed Payment Program balance(s), in full by the due date each month.
Minimum Interest Charge If you are charged interest, the charge will be no less than $1.

How We Will Calculate Your Balance: We use a method called 'average daily balance (including new purchases)'. See your Cardmember Agreement for more details.

How Variable Interest Rates Are Determined: After the introductory period, your interest rate is a variable rate and is determined by a combination of the Prime Rate (which may vary) added to a margin (which does not change). Because the Prime Rate may vary, your variable interest rate will go up or down if the Prime Rate changes. If you are granted an Account, the following rates on the Account are variable: Non-Introductory Purchase Rate; Non-Introductory Balance Transfer Rate; Cash Advance Rate. More information is available in the Cardmember Agreement.

Fixed Payment Programs: From time to time we may offer to you the benefit of our Fixed Payment Programs, including ExtendPay® Plans and ExtendPay® Loans, which allow you to pay off balances in fixed monthly payments over time. See your Cardmember Agreement for more information.

By submitting this application, you understand and agree that U.S. Bank National Association ("we", "us" or "our"), as the creditor and issuer of your Account, will rely on the information provided here in making this credit decision, and you certify that such information is accurate and complete to the best of your knowledge. If we open an Account based on this application, you will be individually liable (or, for joint accounts, individually and jointly liable) for all authorized charges and for all fees referred to in the most recent Cardmember Agreement, which may be amended from time to time. We may request consumer credit reports about you for evaluating this application and in the future for reviewing Account credit limits, for Account renewal, for servicing and collection purposes, and for other legitimate purposes associated with your Account. Upon your request, we will inform you if a consumer report was requested and, if it was, provide you with the name and address of the consumer reporting agency that furnished the report. By providing us with a telephone number for a cellular phone or other wireless device, including a number that you later convert to a cellular number, you are expressly consenting to receiving communications—including but not limited to prerecorded or artificial voice message calls, text messages, and calls made by an automatic telephone dialing system—from us and our affiliates and agents at that number. This express consent applies to each such telephone number that you provide to us now or in the future and permits such calls for non-marketing purposes. Calls and messages may incur access fees from your cellular provider. By submitting this application, you also agree that we may verify your employment, income, address and all other information provided with other creditors, credit reporting agencies, employers, third parties, and through records maintained by federal and state agencies (including any state motor vehicle department) and waive any rights of confidentiality you may have in that information under applicable law. By submitting this application you certify that you read and understood the disclosures here and you agree to the terms of this application.
EXPANDED ACCOUNT ACCESS: Any Card or PIN issued to or selected by you under this Agreement can be used to access multiple checking, savings, line of credit and credit card account(s) held in your name with us or our bank affiliates; and any account you open with us and our affiliates may be accessed by the Card or PIN issued under this Agreement now or in the future. “Expanded Account Access” means use of a card or account number and PIN to conduct a transaction or obtain information at ATMs, over the telephone, through personal computer banking, or via any other available method. If the Card or PIN is for a joint account, the Card or PIN can be used to access all the accounts linked to the Card or PIN Access, whether joint or individual. There are no additional fees or charges for Expanded Account Access, but fees applicable to each applicable account will continue to apply in accordance with the terms of the applicable account agreements. Call Cardmember Service at 800-285-8585 to cancel Expanded Account Access, allowing a reasonable time for cancellation to become effective. If you cancel Expanded Account Access for any account, this Account will not be accessible by any card or PIN other than the Card or PIN issued under this Agreement.
Authorization for the Social Security Administration to Disclose Your Social Security Number Verification: In connection with your application for this account and by clicking the ACCEPT AND SUBMIT* button, you authorize the Social Security Administration (SSA) to verify and disclose to us through Early Warning Services, LLC, our service provider, whether the name, Social Security Number (SSN) and date of birth you have submitted matches information in SSA records. This one-time consent is valid for the next 90 days.

Balance Transfer Information: Balance transfer transactions from other U.S. Bank National Association accounts are not permitted. You may cancel a balance transfer request within 10 days of account opening by calling 800-285-8585. After 10 days from account opening, requests to stop payment on Balance Transfer Check(s) issued by U.S. Bank shall not be honored unless the check(s) has been stolen, lost or destroyed.

U.S. Bank SmartlyTM Visa Signature® Card Rewards Summary

1. Cash back rewards are earned under the U.S. Bank SmartlyTM Visa Signature® Card program. These rewards are earned as “Points”, and U.S. Bank will credit your U.S. Bank SmartlyTM Visa Signature® Card with 2 Points for every $1 in eligible Net Purchases.  You may earn additional Points for a Smartly Earning Bonus if you: 

  • Have or open a U.S. Bank Smartly® Savings account with a minimum opening deposit of $25; and
  • Have “Combined Qualifying Balances” with U.S. Bank in open consumer checking account(s), money market savings account(s), savings account(s), CDs and/or IRAs, U.S. Bancorp Investments and personal trust account(s) (business accounts, commercial accounts, and the Trustee only (IFI) client relationship do not qualify). The Smartly Earning Bonus tiers are:
    • Between $5,000 - $49,999.99, to earn a total of 2.5 Points per $1 (a base of 2 Points plus the Smartly Earning Bonus of 0.5 Points),
    • Between $50,000 - $99,999.99, to earn a total of 3 Points per $1 (a base of 2 Points plus the Smartly Earning Bonus of 1 Point), or
    • $100,000 or more to earn a total of 4 Points per $1 (a base of 2 Points plus the Smartly Earning Bonus of 2 Points).

The Combined Qualifying Balance is determined as follows:
a. First, at the end of each month an average Combined Qualifying Balance is calculated.  The balance is based on a 90-day average balance of all qualifying accounts and is calculated by adding together the end of day ledger combined balances for every day in the last 90 days and dividing the total by

i. 90 calendar days, or
ii. the number of days since opening an initial qualified account.

  • Next, the Smartly Earning Bonus tier that is applied to your U.S. Bank Smartly credit card account is determined by one of two potential calculations:

    i. For the initial evaluation after opening your first qualifying account and at the beginning of every calendar month thereafter, we will apply the Smartly Earning Bonus tier based upon the average Combined Qualifying Balance (as described above).
    ii. In addition, we will review the Combined Qualifying Balances on a daily basis to determine whether you qualify for a higher Smartly Earning Bonus tier based upon the day’s Combined Qualifying Balances.

If your daily Combined Qualifying Balances qualify you for a tier upgrade, you will be moved to the higher tier (within 5 business days).   You will remain in that tier until you qualify for a higher tier based on your daily Combined Qualifying Balances, or the calculated monthly Combined Qualifying Balance qualifies you for a higher or lower tier.

The Smartly Earning Bonus tier will be applied to your U.S. Bank Smartly credit card account within the first 5 days of each month or upon reaching a higher tier, which means that the bonus tier that will be applied to your account may vary during a billing cycle.  The Smartly Earning Bonus is applied as of the Purchase transaction date (which may not be the date the transaction posts to the account).

For joint accounts: The applicable Smartly Earning Bonus tier will be determined by reviewing the Combined Qualifying Balances in the qualifying accounts of each Cardmember (if there is more than one) and using the total Combined Qualifying Balances of the individual whose accounts have the highest total.  (This means that all Cardmembers may be aware of the balance range of the Cardmember with the highest qualifying balance each billing cycle.)  Balances of the Authorized User(s) will not qualify for determining the Smartly Earning Bonus tier. 

Points will expire if there is no reward, purchase, or balance activity on your account for 12 consecutive statement cycles. 

2. This card earns cash back rewards in the form of Points. To redeem as Cash Back in the values noted in this advertisement, Points can be redeemed as a deposit into an eligible U.S. Bank deposit account.  Other redemptions, such as for statement credits and gift cards, may be at a reduced redemption rate.
