What this means for you: When you open an account with us, we will ask for your and/or your business's legal name, address, date of birth, tax identification number, and other identifying information that will assist us with identifying you and/or your business. We may also ask to see your photo identification (driver's license) or other identifying documents. For businesses, we may ask for copies of certified articles of incorporation, an unexpired government-issued business license, a partnership agreement, or other documents that indicate the existence and standing of the entity.
We may change APRs, fees, and other Account terms in the future based on your experience with U.S. Bank National Association and its affiliates as provided under the Cardmember Agreement and applicable law.

Fixed Payment Programs: From time to time we may offer to you the benefit of our Fixed Payment Programs, including ExtendPay® Plans and ExtendPay® Loans, which allow you to pay off balances in fixed monthly payments over time. See your Cardmember Agreement for more information.
Balance Transfer Information: Balance transfer transactions from other U.S. Bank National Association accounts are not permitted. You may cancel a balance transfer request within 10 days of account opening by calling 800-285-8585. After 10 days from account opening, requests to stop payment on Balance Transfer Check(s) issued by U.S. Bank shall not be honored unless the check(s) has been stolen, lost or destroyed.
Cash back is only earned on eligible Net Purchases. Net Purchases are Purchases minus credits and returns. You will earn 3% cash back for every $1 of the first $4,000 spent annually on Purchases classified as insurance, starting with your January statement and 1% cash back after that. “Annually” is defined as the period of time starting with your January statement (which may include transactions made in December) and running through your December statement (which may not include all transactions made in December). You will earn 2% cash back for every $1 spent on Purchases at merchants classified as gas stations, electric vehicle charging stations, restaurants, fast-food restaurants, bars, grocery stores, food delivery services and drug stores. You will earn 1% cash back for every $1 on all other eligible Purchases. Purchases at or delivery from discount/retail stores that sell groceries may not qualify for the grocery store or grocery delivery category. Each merchant’s business is identified by a category code established by Visa. A Purchase will not earn cash back if the category code applied to a merchant is in a category that is not eligible for cash back earning. We do not determine the category codes applied to merchants’ businesses and reserve the right to determine which Purchases qualify for additional cash back. Not all transactions are considered to be Purchases and eligible to earn rewards, such as transactions posting as Convenience Checks; Balance Transfers; Advances (including ATM withdrawals, wire transfers, traveler's checks, money orders, foreign cash transactions, betting transactions, and lottery tickets); interest charges and fees; credit insurance premiums; and transactions to (i) fund certain prepaid card products, (ii) buy currency from the U.S. Mint, or (iii) buy cash convertible items. Upon approval, see your Cardmember Agreement for details. Account must be open and in good standing (not past due or overlimit) on the closing date of the billing cycle in order to earn and redeem cash back. Cash back is redeemed in the form of a statement credit or a deposit made to your U.S. Bank savings or checking account, or as a Rewards Card ($25 minimum redemption). Subject to applicable law, cash back rewards will expire at 11:00 p.m. Central Time at the end of the calendar quarter, that is three (3) years from the date on which they were earned.
As a condition to obtaining a State Farm® Cash Rewards Secured Visa Credit Card (the "Credit Card Account"), I hereby grant to U.S. Bank National Association ("you") a security interest in, and assign and transfer to you all rights, title, and interest in my Secured Savings Account (the "SSA") held at U.S. Bank National Association and all interest, additions, and proceeds therein to secure the payment and performance of my obligations to you under the terms of the Credit Card Account Cardmember Agreement. I hereby instruct U.S. Bank National Association to open the designated SSA in the name of "U.S. Bank (Primary Applicant's name) Secured Credit Card Collateral Account" using the Primary Applicant's name and Social Security Number or Taxpayer Identification Number provided on this application. I agree that while this security interest is in effect, you will have the exclusive dominion and control and you will have the sole right and power to redeem, collect, and withdraw any part or the full amount of the SSA. If I am in default under the terms of my Credit Card Account Cardmember Agreement, I agree that you will have all rights the law allows, including the right to take funds from the SSA and apply them to my Credit Card Account balance without notice to, or further consent from, me. I understand and agree that, based upon your periodic review of my overall credit history (including this Credit Card Account and other credit cards and loans), I may qualify to have the funds in the SSA released. I understand that if you determine that the funds from my SSA may be released, you will release the funds to the Cardmember(s) and this Credit Card Account will no longer be a secured credit card account.
