IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT: To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify and record information that identifies each person who opens an account. What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.

We may change APRs, fees, and other Account terms in the future based on your experience with U.S. Bank National Association and its affiliates as provided under the Cardmember Agreement and applicable law.

How We Apply Your Payments:  We apply your payments to balances with lower APRs first.

Interest Rates and Interest Charges U.S. Bank Business Cash Rewards Card
Annual Percentage Rate (APR) for Purchases Business Cash Rewards Card: 0% introductory APR for the first 12 billing cycles. After that,
13.99% to 22.99% based on your creditworthiness when you open your account.
This APR will vary with the market based on the Prime Rate.
APR for Balance Transfers Business Cash Rewards Card: 0% introductory APR for the first 12 billing cycles for balances transferred within 30 days from account opening. After that,
13.99% to 22.99% based on your creditworthiness when you open your account.
This APR will vary with the market based on the Prime Rate.
APR for Cash Advances Business Cash Rewards Card: 25.99%
This APR will vary with the market based on the Prime Rate.
Penalty APR and When it Applies 29.99%
This APR may be applied to your account if you:
1) Make payments 5 calendar days late twice or 30 calendar days late once; or
2) Make a payment that is returned; or
3) If you exceed your Credit Limit 2 times in 12 consecutive months.
How Long Will the Penalty APR Apply? If your APRs are increased for any of these reasons, the Penalty APR will apply until you make 6 consecutive minimum payments when due and do not exceed your Credit Limit during that time period.
How to Avoid Paying Interest on Purchases Your due date is 24 - 30 days after the close of each billing cycle. We will not charge you interest on purchases if you pay your entire balance by the due date each month.
Minimum Interest Charge If you are charged interest, the charge will be no less than $2.

How We Will Calculate Your Balance: We use a method called 'average daily balance (including new purchases)'. See your Cardmember Agreement for more details.

Loss of Introductory APR: We may end your introductory APR and apply the standard APR or Penalty APR if you make a late payment, make a payment that is returned, or your account exceeds its Credit Limit.

How Variable Interest Rates Are Determined: After the introductory period, your interest rate is a variable rate and is determined by a combination of the Prime Rate (which may vary) added to a margin (which does not change). Because the Prime Rate may vary, your variable interest rate will go up or down if the Prime Rate changes. If you are granted an Account, the following rates on the Account are variable: Non-Introductory Purchase Rate; Non-Introductory Balance Transfer Rate; Cash Advance Rate. More information is available in the Cardmember Agreement.
Notice to Ohio Residents: The Ohio laws against discrimination require that all creditors make credit equally available to all creditworthy customers, and that credit reporting agencies maintain separate credit histories on each individual upon request. The Ohio Civil Rights Commission administers compliance with this law.
Applicant Statement- Business Owner authorizes U.S. Bank National Association (“we”, “us”, or “our”) to obtain a consumer credit report and a business credit report for use in assessing his/her personal creditworthiness in connection with an application by Company, of which Business Owner is an employee, principal, owner, partner, officer, or guarantor, for a U.S. Bank Business Cash Rewards Card. We need such consumer reports because Business Owner may have direct, contingent, present, or future liability to us for the Company’s obligation in connection with the Account. All applicants agree that, as long as the Account is open, we may obtain credit reports about the applicants from time to time.  We reserve the right to consider the applicant for a lower line of credit if one was requested.  This application must be signed by a Business Owner with authority to bind the Business to the terms of this Application Agreement.  The Business Owner certifies that the execution, delivery and performance of this Application has been authorized by all necessary corporate action by the Business, and will provide evidence of such action upon request.  If the Business is approved for the U.S. Bank Business Cash Rewards Card, the Business Owner requests and directs us to open a U.S. Bank Business Cash Rewards Card (“Account”) and to issue U.S. Bank Business Cash Rewards Card (“Cards”) to any individual Employees of the Business, including the Business Owner, designated by the Business Owner on this Application or its addendum, or by any process agreed to by us and the Business.  The Business Owner and the Business shall be jointly and severally liable for all charges on the Account.  Each applicant understands and agrees that we may increase or decrease the APR or credit limit assigned to the Account and/or to the Cards within the Account or close the Account at any time based on our credit guidelines, credit report information, Account history, or the financial circumstances of the Cardmember.  At the time the Account is opened, individual Employees, including the Business Owner, will be issued Cards and a U.S. Bank Business Cash Rewards Card  Cardmember Agreement governing individual use of the Account and individual Employee liability for charges to the Account.  By providing us with a telephone number for a cellular phone or other wireless device, including a number that you later convert to a cellular number, you are expressly consenting to receiving communications—including but not limited to prerecorded or artificial voice message calls, text messages, and calls made by an automatic telephone dialing system—from us and our affiliates and agents at that number. This express consent applies to each such telephone number that you provide to us now or in the future and permits such calls for non-marketing purposes. Calls and messages may incur access fees from your cellular provider.  Use of the Card or the Account will signify acceptance of the terms of the Cardmember Agreement, which may be amended from time to time.  All applicants must be at least 18 years old and agree that Accounts will be used primarily for business purposes, and not personal, family, or household purposes.  Information from this Application may be shared with our affiliates.  Cash access is subject to credit approval.  You certify that to the best of your knowledge, the information provided about yourself, the name and address provided for the legal entity customer, and the information provided about the beneficial owner(s) and/or the individual with control over the legal entity customer is complete and correct.
Cash Rewards Program Information

(a) You may only earn cash rewards (“Rewards”) for Net Purchases on your Account. “Net Purchases” are determined by totaling all new Purchases charged to the Account during a billing cycle, then subtracting credits posted to the Account during the same billing cycle for returns, chargebacks, and/or other adjustments. We may round Net Purchases up or down to a whole dollar amount for purposes of determining and awarding Rewards. Rewards will not be earned for Advances (including wire transfers, traveler’s checks, money orders, foreign cash transactions, betting transactions, lottery tickets and ATM disbursements), Convenience Checks, Balance Transfers, INTEREST CHARGES and Fees, credit insurance charges, transactions to fund certain prepaid card products,  U.S. Mint purchases, or transactions to purchase cash convertible transactions.

(b) You will earn a Reward of 3% on Net Purchases made at merchant locations that are classified in one of the following merchant category codes: Cellular, Gas and Office Supplies. All other eligible Net Purchases will earn a 1% Reward.  We cannot control how merchants choose to classify their business and reserve the right to determine which Purchases qualify. As used herein,(i) the term ”Cellular” means cellular telephone equipment as well as cellular service; (ii) the term ”Office Supplies" means those goods and services offered for sale by a retailer designated with an Office Supply, Business Supply, Printing Supply or a Commercial Furniture merchant or industry code, established by the credit card associations and the merchant; and (iii) the term ”Gas” means and is limited to fuel dispensed for use in a motor vehicle designed for travel on public roads. Fuel dispensed at a warehouse club, superstore, supermarket or convenience store is not eligible for the 3% earn Reward and instead will earn at the 1% Reward. Purchases of gasoline in excess of $200 will not be deemed to be a purchase of automobile gasoline and as such will earn a Reward of 1%.

(c) Accounts that are eligible to earn Rewards are open, current and otherwise in good standing. Accounts are ineligible to earn Rewards when closed and/or after your enrollment in the Cash Rewards Program (the “Program”) is cancelled. At least the Minimum Amount Due must be received by the Payment Due Date or Rewards earned on the previous month’s Net Purchases will be forfeited.  We, in our sole discretion, may disqualify you from participation in the Program and invalidate all Rewards for abuse, fraud or any violation of the Program.

The total amount of Rewards that may accumulate in a 12 month calendar cycle for your Account is unlimited. Rewards accumulated are non-expiring, subject to certain conditions set forth herein. Rewards you earn in a given billing cycle will be reflected on your Account Statement.  Accrual of Rewards may begin on the date your Card and Cardmember Agreement are received by you.  No retroactive Rewards will be given for prior purchases.

(d) Provided your Account is open, current and otherwise in good standing, an annual bonus equal to 25% of your prior year’s Rewards, up to a maximum of $250, will be awarded following your Rewards Enrollment Anniversary Date.

(e) Rewards may be redeemed subject to a $25 minimum and in additional increments of $25.  Subject to Section (g)   below, Rewards that are redeemed will be issued at your discretion in either the form of: (1) a deposit into a U.S. Bank Savings, Checking, or Money Market Account; (2) a U.S. Bank Rewards Card; or (3) a statement credit to your Account. The Account must be open, current and otherwise in good standing to be eligible to redeem the Reward. If your Account is closed for any reason or inactive for 18 consecutive months, or if you fail to make the minimum payment due by the payment due date for two consecutive billing periods, any Rewards remaining on your Account will be forfeited.

(f) We reserve the right to change or cancel the Program at any time, with or without notice to you.  You are responsible for determining any tax liability arising from your participation in the Program.  Consult a tax advisor concerning any tax consequences.  Redemption of any Rewards does not affect your obligation to pay your Minimum Amount Due.

(g) The Business may request that Rewards be pooled to one Rewards balance at the Business level.  The Business individual, who executed the application for credit, will have the sole authority to redeem the pooled Rewards.  If the Rewards balance changes from pooled to individual, any unredeemed Rewards will stay pooled and will not transfer to the individual Rewards balance.
